Your first visit
During your first appointment at Allen Chantry DDS Inc., a thorough examination will be completed. During this consultation, we will discuss your diagnosis and treatment options. In more cases, treatment can be performed the same day as the consultation. Depending on your medical history and the complexity of your condition, a 2nd appointment may be required.
Please provide the following information:
- Your referral slip and x-rays (if applicable)
- A list of your current medical prescriptions
- If you have medical or dental insurance, bring the necessary completed forms
Flexible scheduling is available
Hours of operation:
Monday – Thursday 8:00am – 5:00pm
Closed for lunch 12:00pm – 1:30pm
Every other Friday 8:00am – 12:00pm
If you have current x-rays, they can be forwarded to our office. If you have an emergency, we will do our best to see you the same day.
Your satisfaction comes first
We understand how valuable your time is. We will do our best to stay on schedule. Unfortunately, emergencies do arise and can cause delays. We appreciate your understanding and patience.
Please note, if you are under the age of 18, a parent or guardian must accompany you.
